An elderly couple in Queensland have died after their home elevator malfunctioned.

According to a Queensland Ambulance spokesman, the woman in her 7-s and man in his 80s were at their home in Ayr in the state's north.

It's understood the lift suffered a malfunction, which threw the couple onto the concrete floor below and causing them head injuries.

They were transported from Little Crescent to Townsville University Hospital in a critical condition. Despite the best efforts of physicians, they sadly died overnight.

An investigation has been launched, with Queensland Police and Workplace Health and Safety coming together to workout what went wrong.

The Ayr Criminal Investigation Branch is also at the scene and is combing through the area for evidence and a report will be prepared for the coroner.

Tragic accidents around elevators have occurred in the past.

A woman who was crushed to death by a lift was said to be carrying a large box at the time which hit a switch and sent the lift crashing down two months ago.

Carrie O'Connor, a lecturer at Boston University, is reported to have been dragging the 2m, 36kg box into the lift when it plummeted to the first floor.

The 38-year-old's body was found in the lift following the incident which took place on 14 September at an apartment building in Boston in the US.

According to a report written by Massachusetts Office of Public Safety and Inspections Inspector, Martin Guiod, the lift didn't malfunction and the box had hit a switch that indicated that the door was closed, but it was not.

Inspector Martin Guiod said the death was a 'tragic convergence of circumstances'.
