The 48-year-old actor started his own tequila brand this year, and sent two bottles of his Teremana bevvy to Marie Grover.
Marie, who turned 101 this month, even received a personalised birthday card from The Rock, who also wished her a happy birthday last year after a tweet from Jamie Klingler, who asked if the former WWE wrestler could say happy 100th birthday to her best friend's grandmother.
The centenarian was visibly overjoyed at the present, with Dwayne sharing a video of himself speaking to her and her mate.
Speaking on video chat, he said: "Marie Grover, I can't believe it, singing this birthday song to you is such an honour.
"It is my privilege and pleasure because you are turning 101 today."
He continued: "Happiest of birthdays to you. What an incredible life! I hope you have the best birthday surrounded by love... sending you so much love, what a beautiful 101 years."
Both of the women were giggling throughout the video, clearly made up by a surprise appearance from the man himself.
The pair were wisely advised by Dwayne not to do shots, but instead to sip the drink. Well, fair enough.
He captioned the video: "'Oh my God that's not my Rock is it?'
"I surprised #GrandmaGrover for her 100th birthday last year and I'm HONOURED to surprise her again for her amazing 101st birthday!
"I love how she starts fixing her hair when she sees me on screen!

He shared a photo yesterday with 'Grandma G', saying: "HAPPY BEAUTIFUL 101 BIRTHDAY to Grandma Grover!
"Good to see you received my gift of @teremana bottles and the card I wrote you! Remember to SIP the Mana, grandma and it's not time to do shots!
"Happiest of birthdays - what an amazing and blessed life. Enjoy your mana, Marie!
